Please fill out the following form if your child has additional needs.
Wanting to discontinue your enrolment?
Enrolments are automatically rolled over each term during the year.
If you decide not to continue classes, we require you to complete a Discontinuing Form. Click on the link below and complete the form and return to HYC office at least 2 weeks prior to your last class.
